Select the link to easily send our team a prewritten email.
Complete the broker partnership application.
Our team will send you a link to fill out the online application. Before you submit your information, please note:
- This broker partnership contract is only for IFP and group.
- Your contracted tax ID number through Covered California™ and through Sharp Health Plan must match to avoid commission discrepancies.
- Your contracting information should match IRS records to avoid discrepancies.
- Please provide the most updated information and contracting documents.
- The broker partnership contract will have an effective date on the first of the month after the broker partnership form is received.
Complete the required agreements.
After submitting the application, you will receive a broker agreement, business associate agreement and an Addendum via Docusign within 3-5 days. Revise y firme o ponga sus iniciales donde corresponda.
After receiving the signed broker agreements, we will review your application.
